Form A:
Lobbyist Registration Form

Prior to conducting any lobbying, the lobbyist shall insure that all principals file a Form B, signed by the principal or the principal’s representative, stating that the lobbyist is authorized to represent the principal and that the principal has not offered to pay or give a contingency or success fee to the lobbyist.

(Such issue shall be described with as much detail as is practical, including, though not limited to a specific description ( where applicable) of a pending ordinance, resolution, or a given item on the agenda.)

Provide the existence of any direct or indirect business entity relationship the lobbyist and the principal have with any Covered Persons of the City as that term is defined in Section 17.3. (1)(a) of the City’s amended Charter or their immediate family members: (attach additional pages if necessary)

The above named Lobbyist and/or Principal does hereby agree that any electronic signature appearing on this document is deemed to be the same as a handwritten signature for the purposes of validity, enforceability, and admissibility and shall be deemed to have the same legal effect as delivery of an original executed copy of this document for all purposes.

*Failure to properly register as a Lobbyist can subject violators to penalties as provided in Section 2-979 (g) of the Ethics Code. Repeated offenses can subject the Principal or Lobbyist from doing business with the City for up to 12 months.

** On or before January 31st of each year, every lobbyist must file an expenditure statement (Form D) with the Ethics Commission for the preceding calendar year but only if the lobbyist has incurred expenses exceeding $100.00 per category during the reporting period.

*** A separate lobbyist registration is required for each individual lobbyist even if they are members of the same firm or business entity.
